- This is an excellent opportunity for a physician interested in a full-time position which involves working 50% in a Planetree certified nursing home/sub-acute rehab center and 50% in an academic tertiary hospital based in Queens.
- Collaborate with multi-disciplinary teams in providing comprehensive clinical care for sub-acute rehab patients at The Silvercrest Nursing Home.The team includes advanced practice providers, nursing staff, social work, pharmacy consultants, rehabilitation professionals, nutritionists, spiritual and religious personnel and various specialty consultants in the medicine and surgical fields.
- Conducts palliative care consults along with teaching opportunities for Geriatrics at NewYork-Presbyterian/Queens
